| In order to input your
claim, you will need the following items: |
• Government Bill of Lading
number (also known as GBL#) - This can be found on the upper right-hand corner
of your Bill of Lading. SCAC can be found in box #2 of your Government
Bill of Lading.
• List of loss or damage at or after delivery - Having a list available
will assist you in completing the itemized claim form.
• Complete mailing and physical address, including zip codes. |
